New Patient Appointments

Our staff has been trained to answer many of your questions and will assist you in scheduling your first appointment with our office. You will be asked for the following information when scheduling the appointments:

  • Reason for your visit
  • Name of your referring physician if there is one.
  • Insurance information (including ID and Group Number)
  • Whether your plan requires a referral





 


When scheduling you appointment, time will be allowed for completing paperwork necessary for treatment and filing your claim with your insurance carrier. The receptionist will also refer you to the New Patient Registration Packet in the Documents section of our website. This new patient document will ask for information about your past medical history, your medication, insurance coverage, and demographic information. Now that our office has transitioned to electronic medical records, it is crucial to have as much information entered into our system prior to your arrival. We will ask your assistance, whenever possible, for you to download these forms, complete them, and fax them to us (281) 762-6339 prior to your visit to minimize any delays in adding this into our system after you arrive.

We will also ask you to bring a picture ID (Driver's License) and your insurance card to your visit so we can scan this information into your electronic chart.

Part of your exam will be a review of your medical information with Dr. Stein. He will discuss your medical history and then perform a comprehensive physical examination, after which he will discuss with you his impressions and recomendations. At the end of your visit, any necessary procedures or testing will be discussed.
